About
Our Practice

At Mind & Body Family Health, we believe healthcare should be personal, accessible, and built on trust. We are a nurse practitioner-led practice offering comprehensive, patient-centered care with a focus on prevention, long-term wellness, and empowering patients to take control of their health.

Our model emphasizes quality over quantity. No rushed visits, no corporate red tape, and no barriers between you and your provider. Whether you're managing a chronic condition, seeking hormone therapy, pursuing weight loss, or simply staying on top of your health, we're here to support you every step of the way.

We offer direct access to your provider, flexible scheduling, and a warm, welcoming environment where you’re treated like a person, not a number.

Compassionate care. Clinical excellence. Real relationships.

Two women standing together outdoors in a grassy park during sunset, with trees in the background. One woman has shoulder-length brown hair, tattoos on her right arm, and is wearing a black shirt and ripped jeans. The other has long brown hair, and is wearing a green sweater and ripped jeans. Both are smiling.

Megan Bade, FNP-C

I started my nursing career in a nursing home working with geriatrics, which gave me a strong appreciation for compassionate, long-term care. From there, I moved into the hospital setting on an acute medical floor, where I gained critical experience, especially while working through the toughest parts of the COVID-19 pandemic.

After that, I spent some time travel nursing, which gave me a broader perspective and helped shape the kind of provider I want to be. All of those experiences led me to pursue my FNP and take the next big step in my career.

I’m especially passionate about the direct primary care model because it allows me to truly get to know each patient on a deeper level. It gives me the time and space to explore what's really going on and dig into the root cause of symptoms, rather than just treating surface issues.

I’m excited to open this new practice and continue growing as a Nurse Practitioner. Outside of work, I love spending time with my family, we enjoy camping and watching my girls play sports. I also compete in Strongman competitions, which has taught me a lot about discipline, resilience, and pushing limits, lessons I carry into my practice every day.

This is a brand-new chapter, and I couldn’t be more excited for what’s ahead!

*This membership focuses on medical weight loss and metabolic health, including GLP-1 medications when appropriate, hormone balance, nutrition guidance, and on going support.

*Please Note: The Metabolic and Hormone Health plan is designed to address weight and hormone concerns together (including check‑ins, medication management, nutrition, fitness guidance, etc.). It does not cover all primary care services (such as sick visits, chronic disease management, annual physicals, or preventive care). Those services are included under our Individual Primary Care Plan.

  • Focused Care Visit $150

A single visit with our primary care provider for patients who do not need ongoing membership or follow-up care. Please note: this visit does not establish continuing care with our clinic. Ongoing management, chronic disease follow-up, and refill requests require enrollment in our primary care services.
