It does not cover all primary care services (such as sick visits, chronic disease management, annual physicals, or preventive care). Those services are included under our Individual Primary Care Plan.

A single visit with our primary care provider for patients who do not need ongoing membership or follow-up care. This appointment includes a comprehensive evaluation of your current concern, review of medical history, physical examination as needed, and discussion of treatment options. Prescriptions, lab orders, or referrals may be provided when appropriate.

Please note: this visit does not establish continuing care with our clinic. Ongoing management, chronic disease follow-up, and refill requests require enrollment in our primary care services.
